For First Time Adult Application

• Driver's License or Health Card

• Original Birth Certificate or Canadian Citizenship Certificate/Card

• A Guarantor: a person with a valid Canadian Passport who has known you for two years

• Passport Photos (which can be obtained from Staples, Shoppers Drug Mart, and CAA)

For Children's Applications

• Child's Long Form Birth Certificate

• Passport Photos

• Both parents MUST be present to sign the application

• A Guarantor: a person with a valid Canadian passport who has known the child for two years 

A Guarantor must do the following prior to submitting the application:

• Fill out and sign the guarantor section of the application

• Sign the back of one of the 2 passport phots

• Sign and date a photocopy of your driver's licence or health card (both front and back)

For Simplified (Renewal) application: 
If passport has been issued within the last 15 years

• New Passport Photos

• Current Passport

• 2 references (they cannot be family members).
